50-year-old Todd Parfitt was piloting a single-engine plane with his daughter, 14-year-old Nicole, when experts say a combination of his own inexperience and mismanagement of the control systems caused the plane to crash, killing both of them. The Federal Aviation Administration’s investigation concluded that the plane spun as a result of these errors, and cited that Parfitt’s last flight log was from May, 2012, which likely compounded the issue of inexperience.
